On-call services:
We prefer to handle routine questions, concerns and conditions during our office hours and urgent care clinic. However, for after hours emergencies that can not wait until regular office hours, you can speak to our on-call physician by calling (801) 929-0409.
It can take up to 48 hours to process referrals, so please plan ahead.
Many insurance companies require a referral from a patient's primary care physician before they will authorize care from a specialist. We see these referrals as an opportunity to facilitate your care with the best providers on your insurance panel. If your particular insurance plan requires you to choose a primary care physician (either within Olympus Clinic or outside) and requires a referral before authorizing services to a specialist, please notify us prior to your visit, so that we may assist you in obtaining a referral. We are committed to assuring you the right level of care from the appropriate provider in a timely manner.

Prescription refill policy

Your physician will provide you with enough prescription refills to last until you need a follow-up appointment. We feel that this enable us to provide a greater level of care. Please schedule an appointment before your prescription is out so you will no be without your medication.
We will not accept prescription refill requests over the phone or by fax from your pharmacy.

Most basic medical conditions can be treated at the clinic. We have an in-house x-ray and laboratory, and can handle most lacerations, fractures, sprains, infections, etc. However, for all life threatening conditions, please go to the nearest emergency room.
- Cancellation of an appointment must be made 24 hours in advance to avoid being charged for a missed appointment.
- New Patients without insurance or proof of insurance will be required to pay a $75 dollar co-pay at the time of their visit. The $75 dollars will be applied to their bill for that date of service.
- Established patients with no insurance will be required to pay a $40 dollar co-pay at the time of their visit. The $40 dollars will be applied to their bill for that date of service.
- Patients filing under Workers Compensation or Accident are still responsible for their bill and will be required to pay a $40.00 co-pay at time of service with proof of claim form or $75.00 with out proof of claim form. You will be billed for any outstanding balance and provided with insurance claim forms to submit your own billing.
