 On your first visit, please plan to arrive fifteen to twenty minutes before your scheduled appointment to complete medical history and registration forms, so that we can begin a permanent medical record for you.
You can also pre-register online.
Please bring your insurance identification cards and any medications you are taking. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.
We try to accommodate as many appointments as possible each day. Please make every effort to keep your scheduled appointment, or let us know at least 24 hours in advance if you must cancel.
